Children’s Sleep Habits Questionnaire
The Children’s Sleep Habits Questionnaire (CSHQ) is a sleep questionnaire by Dr. Judith Owens for children aged 4 to 12 years, designed to screen for the most common sleep problems in that age group. The CSHQ focuses on sleep disorders common to this age group in three domains: Dyssomnias (difficulty getting to sleep or staying asleep), Parasomnias (sleepwalking/talking, night terrors, bedwetting, restless leg syndrome, etc), and Sleep-Disordered Breathing. A score of 41 or higher on the abbreviated CSHQ is considered by the authors to be an indicator for referral to a sleep specialist.
The CSHQ is not intended to be used to diagnose specific sleep disorders, but rather to identify the need for possible further evaluation. Reliability and validity data has been collected on a sample of 495 elementary school children and on a clinical sample from a pediatric sleep clinic. It would not seem valid to use this scoring cutoff for newly arrived adopted children given all of the transitional issues, but this questionnaire could certainly be useful in discussing sleep problems with your provider.

Have you tried the author? I don't have a scoring tool for that one. They seem to have a computerized scoring program that might apply, or other specific scoring instructions for the Adolescents questionnaire. I don't think it's validated for preschoolers. I'd recommend you contact the authors (see above).

As for the "what questionnaire should I use for my research" questions, I'd recommend you contact the actual authors and sleep specialists, not the guy who blogged about their survey.
